For the next step we request the Account Holder complete some simple documentation to help protect the account from unauthorized use.

The documents may be digital pictures or scanned for upload.
The documents may also be faxed. All we need is the following:

  • Completed Sworn Claim Affidavit & Proof of Loss Statement
  • Copy of a Government Issued ID e.g. Drivers License, Passport
  • If required, copy of the most recent wireless bill for this mobile number
  • If required, copy of Proof of Purchase for the claimed device e.g. Receipt, UPC Code From Box
  • If required, copy of supporting documents e.g. police report, insurance document, etc.
